**Root cause:** Doc read [FLEET/INVENTORY.md](FLEET/INVENTORY.md) directly without LAGOS-grep on GOG1 broadly. INVENTORY.md was stale (last-corrected 041726). Doc operated on partial-substrate-snippet and projected stale-state forward.

**Per Doc CLAUDE.md Rule 12 LAGOS-SCOPE-COLLAPSE PROCEDURAL-GATE** failure-class: single-box query (Bridge grep on FLEET/INVENTORY.md only) when substrate-wide intent (GOG1 current-operational-status) was intended. Rule 12 did NOT fire on Fleet-PMFST composition because Doc treated INVENTORY.md as canonical-of-record without cross-substrate-verification.

**Root cause:** Doc routed verification-tasks back to Dan despite explicit prior correction in RAM-059 §A self-catch on same pattern. Doc CLAUDE.md Hard Rule #5 grep-before-send mental-track did NOT fire on the verification-task-routing language.

**Pattern:** Doc creates Dan-class to-do-list framing → role-reversal → Dan corrects → Doc absorbs → Doc recurs.
